Booking & Appointment Policy Certified DOT Specimen Collection and Logistic Solutions Effective Date: October 28, 2026 At Certified DOT Specimen Collection and Logistic Solutions, we are committed to providing professional, reliable, and convenient drug testing, alcohol testing, DNA testing, specimen collection, and related services. To ensure efficient scheduling and quality service for all clients, please review our Booking & Appointment Policy before scheduling an appointment. 1. Appointment Scheduling Appointments may be scheduled through our online booking system or by contacting our office directly. We request that appointments be scheduled at least 24 hours in advance. Same-day appointments may be accommodated based on availability. Appointment availability varies by service, location, and collection personnel. Certain services may require additional information, documentation, or preparation before an appointment can be confirmed. Mobile and on-site testing appointments require coordination and confirmation by our team. Submitting an online booking request does not guarantee an appointment until confirmation has been provided. 2. Appointment Confirmation Once an appointment is scheduled and accepted, clients may receive a confirmation by email, text message, or other designated communication method. Clients are responsible for reviewing their appointment date, time, location, and selected service. If any information is incorrect, please contact our office before your scheduled appointment. 3. Payment Policy Payment requirements vary depending on the service selected. Certain services may require full payment at the time of booking. Other services may require a deposit or payment at the time of service. Any applicable additional fees will be disclosed before they are charged. Employer-sponsored testing may be billed according to an established business agreement. Clients are responsible for selecting the appropriate testing service and providing accurate payment information. All payments are processed through our designated payment system. 4. Cancellation Policy We understand that schedules may change. Clients are encouraged to provide at least 24 hours' notice when canceling an appointment. Cancellations may be submitted through the booking system, when available, or by contacting our office directly.
